Warning Signs of Suicide: Free Printable Checklist for Schools

When students exhibit warning signs of suicide, they are at an increased risk for suicide within a short period of time—often within days, hours, or even minutes.

This printable checklist is adapted from Hazelden Lifelines Trilogy, a school-wide suicide prevention program, to help teachers identify warnings signs of suicide.
